EXPANSION OF ADULT DAY HEALTH CARE OUTREACH PROGRAM PROVIDES
CAREGIVER SUPPORT

 

 

“CAREGIVER SUPPORT AND EDUCATION”


The Canandaigua VA Medical Center has plans for expansion of a recently implemented program to provide Adult Day Health Care (ADHC) to our rural, and now our Monroe County Veteran population. Working with our Veterans Service Organization community partners in Monroe County, two more locations will be served by a team of traveling VA caregivers:

In addition to:


• Warner - Van Riper American Legion  Post 435 - Waterloo, NY  - every Monday
• Johnson - Costello American Legion  Post 355 - Penn Yan, NY - every Wednesday

Two new locations added:


• Cottreall-Warner American Legion Post 942 Webster, NY, has offered use of their facility every Tuesday beginning 11/9/10
• American Legion  Greece Post 468 Greece, NY, has offered use of their facility every Thursday, (December opening date to be announced)

This program will be held weekly, at each site.

The care provided in this ADHC program will be consistent with a social model adult day program.  Consistent with VA directives pertaining to delivery of VA-based Adult Day Health Care, the mobile team will coordinate services to include:


 Individualized plan of care
 Coordination with primary care provider
 Case management
 Therapeutic exercise
 Leisure activities
 Weight and blood pressure monitoring
 Personal care
 Therapeutic meal
 Therapeutic pet visits
 Programming for dementia patients
 Caregiver support and education
 Activities provided by Volunteer Services
